I understand that North County Christ the King Church (“North County”) is relying on the accuracy and completeness of the information I provide on this application.

I attest and affirm that all the information I have provided is true, correct, and complete to the best of my knowledge.

I voluntarily authorize North County to contact any person or entity listed in my application for the purpose of verifying the information provided and assessing my qualifications for volunteering.

I release North County and any such person or entity from any liability resulting from the disclosure or use of this information in the volunteer selection process.

I understand that I am entitled to a complete and accurate disclosure of the nature and scope of any consumer report on me. I also understand that I may receive a written summary of my rights under 15 U.S.C. § 1681 et. seq.

I understand that I am offering my time and service voluntarily and without expectation of compensation or employment benefits, including insurance coverage. My volunteer role does not create an employment relationship with North County.

I acknowledge and accept that volunteering may involve physical activities that carry inherent risks of injury. I voluntarily assume all such risks, including injury, illness, or other harm, that may result from my participation.

Except in cases of gross negligence or willful misconduct by North County, I agree to be solely responsible for any personal injury or property damage that I may suffer while volunteering. I hereby release, indemnify, and hold harmless North County, its pastors, officers, employees, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to my participation as a volunteer.

I understand that this agreement shall be governed by the laws of the State of Washington. If any provision of this agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I grant to North County the right to make photo, video, and audio recordings of me.

("Recordings") as well as the perpetual, world-wide right to record, re-record, copy, edit, reproduce, publicly display, perform, and distribute the Recordings in any way consistent with North County's ministry, including use on the Internet. I waive the right to inspect or approve the finished product wherein the Recordings appear.

Please note the following release does not apply to any personal, professional, or other project/work that is unrelated to North County.

Any work that I have created in my role/capacity as a volunteer for North County in the past, present, or future, recorded in any form ("Creative Work") is for the exclusive ownership of North County and is considered a "work made for hire" (defined in the US Copyright Act). If, for any reason, the work related to my volunteer service for North County is determined at any time not to be a "work made for hire," I agree to assign, and do assign, to North County all right, title, and interest to the Creative Work.

North County wishes to maintain in confidence all information including data, commercial, and strategies, and know-how disclosed by North County to me, directly or indirectly, in written, oral, or other tangible forms.

In this regard, I agree that I will not disclose the confidential information and/or trade secrets with others without prior written consent of North County both during or after my volunteer service.
